• Helps individuals maintain current levels of functioning and independence
• Provides meaningful opportunities for socialization and peer support via fun activities and outings
• Prevents or delays premature nursing home placement
• Provides assistance and respite for families and caregivers
• Provides assistance with the transition home after a stay in an assisted living or hospital facility
• Offers comprehensive medical assessment and coordination of care with the individual’s physician
• Provides nursing and personal care
• Provides medication monitoring
• Features nutritious meals that observe dietary restrictions
• Offers comprehensive mobility assistance and exercise classes
